Enroll before the September 15 deadline to earn thousands in CMS incentives.
Primary care providers are uniquely positioned to deliver outstanding, patient-centered care to their patient population — and CMS has developed a payment model that closely aligns with these unique capabilities and care processes.
The Comprehensive Primary Care Plus (CPC+) is an advanced primary care medical home model that offers incentives for delivering high-quality primary care, and has been dubbed the “largest-ever” move to change US primary care delivery and payment.
The first CPC+ performance period of begins January 1, 2017, and practices in 14 regions throughout the U.S. (below) will need to enroll before the September 15, 2016 deadline with a Certified EHR to be eligible for these new payment options.
Currently CMS has no plan to accommodate new practices after the starting year, and so registering before the deadline is necessary for inclusion in this program. Practices may withdraw from the model at any time.

Elation has pledged to support quality reporting for both Track 1 and Track 2 for the full 5-year duration of the program:
