Last year, Centers for Medicare & Medicaid Services (CMS) announced the Comprehensive Primary Care Plus (CPC+) program as an advanced primary care medical home model that offers incentives to physicians for delivering comprehensive primary care – care that is more accessible, coordinated, and patient-centered.
In January, the program launched for the nearly 3,000 participating practices and recently, CMS released details for Round 2 of the program. Here is everything independent physicians need to know about the program thus far.
What is CPC+?
CPC+ aims to transform primary care through a primary care quality improvement initiative with incrementally advanced care delivery requirements. More specifically, the program will be monitored over 5 years, along 2 tracks with slightly different payment structures, with yearly milestones that progressively create a robust foundation for participating practices to provide improved patient care, and take an analytical approach to medical decision making.
Essentially, practices will undergo transformation to meet the five core goals of the program: 1) Access and Continuity; (2) Care Management; (3) Comprehensiveness and Coordination; (4) Patient and Caregiver Engagement; and (5) Planned Care and Population Health.
CPC+ is currently made up of practices of all sizes and structures in each of the 14 selected regions:
- Arkansas: Statewide
- Colorado: Statewide
- Hawaii: Statewide
- Kansas and Missouri: Greater Kansas City Region
- Michigan: Statewide
- Montana: Statewide
- New Jersey: Statewide
- New York: North Hudson-Capital Region
- Ohio: Statewide and Northern Kentucky: Ohio and Northern Kentucky Region
- Oklahoma: Statewide
- Oregon: Statewide
- Pennsylvania: Greater Philadelphia Region
- Rhode Island: Statewide
- Tennessee: Statewide
The 13,090 primary care providers and the over 1.76 million Medicare beneficiaries patients in their care are uniquely positioned to experience high-quality, patient-centered care — and as part of the program, CMS has developed a payment model that closely aligns with these unique capabilities and care processes.
What’s new since the program launched?
CMS is continuing the program by announcing a new application period for a second cohort of the program to begin in January 2018. Interested practices have the opportunity to submit their proposals to participate until July 13th. Up to 5,500 practices will be accepted across the program.
Why should independent practices care?
CPC+ has been dubbed the “largest-ever” move to change US primary care delivery and payment. Not only will independent practices have the opportunity to receive incentives for delivering exceptional primary care, physicians are laying the groundwork for transitioning to value-based reimbursement, meaning these practices will be set up for success for value-based programs like MACRA.
