Patient Passport: Redesigned for Modern Primary Care

Today, we're launching the redesigned web experience for Elation Patient Passport.
Patient Passport is the secure digital connection between patients and their care team. With this update, patients logging in from a web browser on any device will see a more modern, streamlined experience built around the features they use most.
For practices, this update is designed to make patient engagement easier to support. Current patient accounts continue to work automatically, patients keep their same login credentials, and no action is required from the practice for existing users.
What's available now
The new Patient Passport web experience helps patients get where they need to go faster and with less friction. Patients can now:
-
Log in more easily with improved setup and support for Face ID and Touch ID
-
Find appointments, secure messaging, and health records front and center
-
Attach files when replying to messages with their care team
-
Manage family and caregiver access more easily under one login
-
Cancel appointments and update demographics directly in Passport
-
Use the experience in English or Spanish
These updates reflect a broader redesign of the Passport experience from the ground up, with a focus on simpler onboarding, clearer navigation, and more self-service for patients.
Why it matters for primary care practices
Independent practices need digital tools that compete with larger systems — without the complexity or vendor sprawl.
When patients can log in easily, find the information they need, and complete routine tasks on their own, practices spend less time fielding avoidable support calls and more time focused on care.
The redesign is built to support:
-
Higher patient activation
-
Less staff troubleshooting
-
Better engagement around messaging and appointments
For independent primary care practices, that means a digital experience that works better for both patients and practice teams.
What's next
This is the first phase of the updated Patient Passport experience. In the coming weeks, we expect to roll out updated iOS and new Android mobile apps, with more enhancements to follow.
Future enhancements will include: additional languages, appointment rescheduling, expanded payment options, and more patient self-serve controls.
This redesign reflects our commitment to independent primary care: powerful tools, zero complexity, built for how you actually work.
